You'll need to complete each of them in order to get credit for the course:

Step one: Complete your tracking log.

If your last name begins A-M, email it to James Kelly.
If your last name begins N-Z, email it to Lesa Zuke.

Once you have received the email verification, then…

Step two: Complete, sign and send your CEU form via interoffice mail.

If your last name begins A-M, interoffice mail it to James.
If your last name begins N-Z, interoffice mail it to Lesa.
